Abt Associates: Bold thinkers driving real-world impact
Abt Associates will support Medtronic Philanthropy as the global implementation partner for HealthRise, a five-year, $17-million program that supports community-based demonstration projects designed to expand access to care and management of chronic, non-communicable diseases (NCDs) such as heart disease and diabetes. Worldwide, more than 100,000 people die every day from an NCD, according to the World Health Organization.
HealthRise projects will initially focus on selected communities in Brazil, India, South Africa and the United States. Abt Associates will coordinate the overall global program and each country-level program, which includes engaging experts and stakeholders, identifying and supporting local implementing organizations, and coordinating education and policy efforts based on lessons learned from the country programs.

Projects will begin with comprehensive community health system assessments led by public health experts, community organizations and local leaders to identify barriers and gaps for underserved populations. After the assessments, the project will make grants to local organizations to improve community-based healthcare services that help people living with chronic diseases better manage their day-to-day health. Using a “Continuum of Care” approach developed by Medtronic Philanthropy, grants will primarily be used to support frontline healthcare workers, conduct patient empowerment and education programs, and enhance local stakeholder engagement and policy efforts that will increase access to NCD healthcare.
The Institute for Health Metrics and Evaluation at the University of Washington will monitor and evaluate the projects.
